Responsibility:
In this position you will be stationed in the Emergency Room.
You will be meeting with patients, reviewing their charts, interact with patients/families to provide post-discharge transition of care outreach and linkage to care management resources.
You will be like a discharge planner to prevent readmission of stay.
Experience:
Looking for someone with healthcare experience.
Must be a Certified Medical Assistant or Associate's Degree.
