Role:
The Director of Clinical Excellence is key to the quality, compliance and growth of our network of self-managed care teams.
You are responsible for Clinical Support to the care teams.
Helping the teams operate in a compliant manner, develop scalable and relevant clinical training for both caregivers and nurses and creating a clinical evaluation program.
Day to day responsibilities will include:

Building and scaling the Clinical Supervision operations to support the care teams and their abiity to grow and admit and serve new clients 2.
Ensure compliance with state and any other government regulation, through training, team audits and working with our Engineering team to develop technology features that will identify and notify teams and nurses on compliance issues.

Work with our Medical Director to continuously monitor clinical outcomes and develop our clinical processes and approach and overtime develop data driven risk management capability that will allow the teams to continuously deliver better clinical results.
The Director of Clinical Excellence, as part of the SwiftShift back office team and overall platform support, also helps ensure that teams have the RN resources that they require in order to certify clients, manage Doctors orders and support any other process that requires an RN if the team doesn't have one in place.
Responsibilities and Success Measures:
Support the Set up New Teams - You will work with local individuals to help form pioneer' groups of 3-4 that can start a new team.
You will support the new team in the process of setting up including creating a team service plan, interviewing and hiring the first team members, help the team select clients and open their first cases, support the team through servicing the patients and getting the teams up to speed.
Clinical Support & Supervision - Create a clinical training and competency program that will ensure new hires are proficient in charting and compliance Develop a scalable audit and quality assurance program working with RNs at the location of each team Update and develop clinical processes to ensure that our teams benefit from the latest research and know how as well as most up to date state and federal compliance requirements.
Case Pipeline Management - You will be responsible for scaling up the Clinical Support team to ensure that new and existing cases that need to be opened for start of service are opened without delay and all recertifications are completed to ensurer continuity of service.
Training and Empowerment - We want to provide the best care possible and to do so we need our caregivers to have the best knowledge and tools.
As the Director of Clinical Excellence you will help identify areas of training and development that could help teams succeed, put together the training plans in collaboration with the platform's support.
Platform Development - You will liaise with the engineering team to identify opportunities for improvement in the platform, help get feedback from the caregivers on prototypes and designs, and help implement new features and get initial feedback on releases and any bugs.
Help Drive the Business Vision Forward - This role has exposure across the entire business.
Driving the business forward sharing feedback for ways to improve and living the Swift Shift values when working and communicating with clients and colleagues across the business.
Requirements:
Passionate about Home Care, Outcome based treatment, use of technology in Home Care and Innovation Strategic thinker - can see the wood from the trees, focus on important issues, build culture, make decisions with a long term view and focus on strategic data to drive decisions.
A qualified RN with home care experience pediatric experience preferred Able to coach others to solve problems while maintaining ownership and leveraging data, best practices, focus on creating a learning organization and a safe environment for learning and failing Able to learrn or Knows the home care regulations in and out both at the medicare level and the state level, understands the DOH and payer strategic goals and concerns and is up to date on all regulatory requirements.
