Qualifications:
Post-secondary education or equivalent Knowledge of peer-based services, recovery principles, and homeless services.
Excellent computer, written, oral, and interpersonal skills.
Excellent organizational skills with strong attention to detail.
A clear understanding of services provided by New Roots and Willow Foundation
Responsibilities:
Develop/maintain strong working relationships with area organizations for mutual referral processes.
Develop/maintain strong working relationships with area landlords and housing authorities.
Serve as point person for community resource events Complete case files for reviews and implement follow-up services.
Foster a collaborative and respectful relationship with each client reflective of rights-based practice.
Facilitate mediation between clients when required.
Assist clients in applying for Public Assistance, Social Security benefits, and other entitlement programs.
Assist with transition planning through liaising, consulting, and advocating where necessary with housing providers, mainstream services, and support persons to evaluate current and ongoing needs and locate appropriate and affordable accommodation.
Develop/maintain strong working relationships with area organizations for mutual referral processes.
Develop/maintain strong working relationships with area landlords and housing authorities.
Support clients to attend to their own support needs.
Ensure the facility meets safety and sanitation standards.
Use of inclusive, culturally appropriate language and attitudes.
Follow the employee handbook, policies, and procedures.
Other duties as assigned.
